Managing Gigantomastia – Breast Reduction Using the Medial Pedicle Wise-Pattern Technique

The medial pedicle wise-pattern breast reduction technique was applied in a challenging case of gigantomastia in an 18-year-old woman. A total of 2 kilograms were removed from each breast.

According to medical literature, when the distance from the sternum to the nipple exceeds 37 cm (in this case, it was 49 cm) and the distance from the nipple to the breast root exceeds 13 cm (in this case, it was 15 cm), the chances of preserving the nipple and its sensitivity are generally low. However, due to the patient’s young age, we were determined to make the effort. The outcome was successful on both counts: not only was the nipple preserved without the need for a free nipple graft, but its sensitivity was fully maintained as well.
